Cataract

What a cataract is?
Cataract is a disease associated with clouding of the lens in the eye. Depending on the progression of the clouding, the clouding causes different degrees of visual disturbances. The clouded lens of the eye becomes impenetrables for light rays and prevents a sharp reflection of the retina, due to what the patient’s sight deteriorates. A mild opacification of the lens in an early stage is not registered by the patient who does not experience any symptoms or deteriotation of the vision at that moment. However, an advanced lens opacification causes vision disturbances as well as other symptoms such as, for example, dioptrical changes, increased or decreased sensitivity to light, bleary perception of surrounding environment, blurred perception of objects, double vision and other symptoms. Speed of the lens opacification is individual. Partly it depends on age, gender and other diseases the patient have had. Cataracts can be congenital or acquired (including in consequence of an eye injury), which may occur at any age. According to statistics, this happens mostly in the age of 65-75 years.

Cataract treatment
Currently there have not been developed any drops, medicines, or glasses which would make possible to treat the caractact without surgical treatment. They only method of eliminating cataract is an eye microsurgery. Surgery involves removing the cloudy lens and subsequently replacing it with an artificial lens. Current technologies assure high efficiency - the vision is practically completely restored. Vision is improved since the first day after surgery. In addition, cataract surgery is nowadays one of the safest eye operations at all. Cataract surgery is a standard surgery treatment which requires virtually no serious restrictions on the part of the patient, both prior the surgery itself, and after it.

Cataract removal
In our ophthalmologic center, all medical procedures, including cataract surgery, are performed on an outpatient basis under local anesthesia (using topical anesthesia). The whole process is as follows: the nurse begins to prepare the eye for the surgery ‒ drops numb the eye surface and extend the pupil. Then she disinfects the area around the eyes, the patient's face is covered with a special sterile mask and the eyelids are fixed. The entire operation is carried out under a special microscope. The surgeon applies a painless method ‒ called phacoemulsification. This method allows the effects of ultrasound to fragment the lens nucleus. The core of the lens is fragmented into small pieces which are then removed. The operation is terminated with the subsequent implantation of a new intraocular lens into the eye case. The new lens has the same dioptric power as natural human lens. All steps of the operation are carried out throughout a microincision which is no larger than 2 millimeters. The biggest advantage of this incision relies in the conclusion of the operation, as it does not have to be sewed. Before the operation itself is performed, the patient should undergo a special pre-operative examination and attend a consultation with a specialist-surgeon.

On what depends the choice of the artificial lens?
Basically, there are only two types of intraocular lenses ‒ a monofocal lenses and multifocal lenses. Monofocal IOL (intraocular lens) is a lens with one dioptrical intensity, which provides a possibility of excellent vision without glasses for distance. For getting a clear image of objects close up glasses will be needed. Multifocal IOL provides quality vision both at distance and at medium and close distances. Multifocal IOL is recommended for patients who frequently change distances. Long-term experience of implantation of multifocal lenses confirm satisfaction of people of all ages. Minor obstacle of multifocal IOL may be a slight reduction in contrast sensitivity and a reduction in night and dusk vision. The monofocal lenses are also called accommodating IOLs.
Moreover, in present day accommodating lenses and toric lenses are often applied. The accommodating IOL are monofocal lenses, but their shape allows a a slight movement of the eye, which enables a sharp vision when looking both at close and  further distance. This movement is not percieved by the patient. Thanks to accommodating IOLs patients see sharply into distant and average operating distances. Toric IOL (monofocal), are able to help, in addition to the removal and replacement of cataract lenses, to correct astigmatism.

What do we recommend after the surgery?
After the surgery the patient must follow a precise schedule using drops in the operated eye. Eye drops protect the eye against inflammation and speed up the healing process. We also recommend avoiding physical exercise for 2-3 weeks after surgery, keeping away from dusty environments, swimming pools and saunas. Activities such as walking, everyday issues, work with computers, watching TV, etc. does not make any harm to the operated eye. The complete stabilization of vision in the case of monofocal lenses occurs in 3-4 weeks and in multifocal and accommodating lenses in 10-12 weeks (this period is called neuroadaptation).

Secondary cataract
In some cases, after an operation may occur what is know as secondary cataract or posterior lens capsule opacification. Secondary cataracts can begin to develop in the early postoperative period (a few months after the surgical treatment), but also, for example, in a few years. It should be pointed out that secondary cataract is not the result of a poorly performed operation. One of the main factors for developing secondary cataract is a natural eye pathology. In 5 to 30 percent of patients in several months or years after surgery, due to natural and completely uncontrollable causes, clouding of posterior lens capsule begins to develop gradually, in which is inserted the artificial intraocular lens. There are the same symptoms as in the case of cataract, hence the name of secondary cataract. Treatment of secondary cataract is easy, painless and is done outpatient: this operation is called a capsulotomy. During the treatment the surgeon creates with a laser an incision, and thereby, the vision is being restored. There is no need of other surgical procedures.
